Health Form

Any youth under must have a health form on file in order to participate in any youth activities.  The forms may be filled out and mailed or taken to the Education office at the church.

Permission Slips

We are now making up just one permision slip with one or two months worth of activities listed on it. Just check the event(s) that your youth will be attending, sign it and send it in. You can can do it once, or send one in for each separate event.
